  • Attend, organise and chair meetings of all types, either with you or on your behalf, write minutes, answer questions, express views, advise, summarise and debrief, and plan next steps

  • Initial case review

  • Make home visits to capture your thoughts and parental views via person centred planning tools,  map concerns and plan next steps

  • Complete forms with you or on your behalf

  • Signpost you to relevant legislation and explain what is means in practical/layman's terms 

  • Meet with your child to gather their views, use ' One Page Profile' tools to capture them in the best way for the Local Authority, Health and Social Care to understand. 

  • Guiding you through processes step by step 

  • Explaining legal frameworks, case law and policies 

  • Planning and preparing the Education, Health and Care (EHC) plans and needs assessments

  • Helping you to understand your options and draft your parental views 

  • Accompany and support you in meetings with school professionals and the local authority, take minutes and discuss technical matters on your behalf

Online support

  • Organise, hold and attend TEAMS or ZOOM meetings with schools, care professional involved in your child’s education/care, either with you or on your behalf

  • Hold weekly online coaching and next steps meetings

PfA and Post-16 support

Preparation for Adulthood and Post-16 Support

You know your adolescents' needs are changing but you aren't sure how it all works to ensure they get the support they need  or what to expect post 16.

You might experience service ‘drop off’ when your child reaches adulthood or the cliff edge effect… it shouldn't be this way but often is. 

The Children and Families Act 2014 sets out a range of duties towards young people preparing for adulthood. These include the provision of information, advice and support, working together to achieve outcomes and strategic joint commissioning. 

 

From the annual review of the EHC plan in Year 9, and at each review thereafter, local authorities must include a focus on preparing for adulthood and transition planning must be incorporated in the EHC plan and set out clear outcomes to prepare young people for adulthood. Planning should include:  Support to prepare for higher education and/or employment;  Support to prepare for independent living – this should include ascertaining where the child or young person would like to live (it may be that they do not wish to move into independent living) and local housing options should be explained;  Support in maintaining good health – including effective planning for transition of health services;  Support in participating in society – including mobility and transport and developing and maintaining relationships and friendships. Reviews should identify what support the child or young person requires to achieve the outcomes and aspirations specified in their EHC plan and to prepare them for adult life. 

 

We can guide you in many ways where transition to adulthood support needs are being assessed and your young person is preparing for adulthood (PfA) .
